HIPAA Notice of Privacy Practices

1. Our Legal Duty

Pet Housing Rights and its network of licensed clinicians are required by federal law (the Health Insurance Portability and Accountability Act of 1996, or HIPAA) to maintain the privacy and security of your Protected Health Information (PHI) and to provide you with this Notice detailing our legal duties and privacy practices.

2. Uses & Disclosures of Your Health Information

We use your health information for the following specific clinical purposes:

• Treatment: Disclosing symptoms, clinical questionnaires, and assessments to the evaluating mental health professional to evaluate your psychological impairment and formulate an accommodation determination.
• Healthcare Operations: Conducting internal credentialing audits to ensure our therapists maintain active state licensing and quality standards.
• Verification Communications: With your explicit written authorization, confirming the authenticity and active license status of your ESA or PSD recommendation letter to your landlord or property management office.

3. Protection Against Unauthorized Disclosure

We will never disclose your specific clinical diagnoses, psychotherapy notes, psychiatric session recordings, or confidential medical background to your housing provider or any unauthorized party without your explicit, voluntary written consent.

4. Your Individual Rights Under HIPAA

As a patient, you have the right to:

• Inspect and receive an electronic copy of your clinical intake records.
• Request confidential communications through alternative means or email addresses.
• Request restrictions on certain uses and disclosures of your protected health information.
• Receive a prompt notification in the event of an unauthorized security breach involving your PHI.

5. Filing a Privacy Complaint

If you believe your privacy rights have been violated, you may file a written complaint with our Privacy Officer at privacy@pethousingrights.com or directly with the Secretary of the U.S. Department of Health and Human Services (HHS). We will never retaliate against any individual for filing a privacy complaint.
